WHO CAN APPLY?
Applying for a new passport or child passport for the first time.
You are applying for a minor under the age of 16.
Your passport was issued before you turned 16.
The prior passport was issued more than 15 years ago.
The expired passport is not in your possession .
The current & valid passport is damaged.
In order to apply and receive a United States passport you must follow the instructions as outlined below.

Letter of Authorization |
- Please print 2 copies and sign both with original signature. Our office must instruct you which letter to use based on where your passport will be processed. Call us below in Step 8 to determine this. One letter will go into the government sealed envelope. One must be included outside this envelope when shipped to our location.

Proof of Citizenship - You must provide ONE of the following: |
- Previous U.S. Passport more than 15 years old
- Certified Birth Certificate issued by state, county or city (if born in US)
- Certificate of Citizenship or FS-240, DS-1350 (if born outside the US)
- Certificate of Naturalization

- Valid driver’s license w/ photograph issued more than 6 months ago
- If you do not have a driver’s license or a driver’s license that meets the requirements above then you must supply personal documents.

Evidence of Immediate Travel - Must provide a copy of 1 of the following: |
- Round trip ticket indicating travel to a foreign country
- Travel Itinerary
- Business traveler’s may submit a business letter of expedite, if the above is not available

Complete but do NOT sign the form until you are in front of an Acceptance Agent. Make a photocopy of this application.

Two Passport Photographs: Must be taken by a professional passport photographer. |
- Obtain 2 passport photos from a passport photographer

Fees: Government fees must be paid by check or money order. Our fee can be paid by check, money order or credit card. |
- Government fees: You will write a check to “US State Department” for $127 for adult / $112 for children and pay $30 to the Acceptance Agent.
- The U.S. Vital Documents fee depends on the processing time you choose please see the order form below for the exact fees.

Authorization Process – An Acceptance Agent will verify your identity and witness the signing of your application.

Child passports require both parents and the child to be present. If only one parent is able to go with the child the parent that is not present must fill out a DS-3052 . Click on the link below for this form.
- Before appearing in front of an Acceptance Agent please be sure you have all the required documents:
1. Letter of Authorization (2 Copies)
2. Proof of Citizenship
3. Proof of Identity
4. Evidence of Immediate Travel (2 Copies)
5. DSP-11
6. Two Passport Photos
7. Government Fees
8. U.S. Vital Document Fee
9. Order Form
- Acceptance Agents are found in Passport Acceptance Facilities in local Post Offices (typically a Main Branch Post Office), Court Houses and other government offices authorized to accept passport applications (Not every post office/court house is a Passport Acceptance Facility) Most Passport Acceptance Facilities are open from 9am to 4pm. You may want to call to verify the hours. Click here to search by zip code.
- Do NOT sign the DSP-11 until directed to do so by the Acceptance Agent. You will sign the application after the Acceptance Agent verifies your application.
- Tell the Acceptance Agent that a courier service will deliver the materials to the Passport Agency. This Department of State policy is called “Hand Carrying of Executed Applications”. The policy is detailed on page 31 and 44 of the Acceptance Agent Manual.
- The Acceptance Agent will check your proof of identity, endorse the documents and seal them in an envelope marked: "To be opened by authorized passport officials only." ”Do NOT open the envelope after it is sealed!
- Some Acceptance Agents do not seal a Release Letter in the envelope. In this case, you MUST attach one copy with original signature to the envelope.
- Print your name, departure date and date of birth on the outside of sealed envelope.

- Send the sealed envelope along with extra letter of authorization and order form by FEDERAL EXPRESS ONLY to:

Do not send your information to any of the addresses listed on the order form without prior confirmation by our office. We will instruct you in Step 8 (above) where to send your application when you call our office.
