This process is for adding pages to a valid passport.

If your scheduled departure is in more than 5 days then you are eligible for our Regular processing.

If you are traveling in less than 5 days, click here for Rush Processing.

In order to expedite your additional pages passport application, please follow each of the following steps. An incomplete or incorrectly completed application will delay the issue of your passport. To make the process easier to complete we suggest you print this page and follow the steps in the order presented.

Letter of Authorization

• Please print 2 copies and sign both with original signature. Our office must instruct you which letter to use based on where your passport will be processed. Call us below in Step 6 to determine this.

Submit Your Valid Passport

Evidence of Travel Must provide a copy of 1 of the following:

• Round trip ticket indicating travel to a foreign country
• Travel Itinerary
• Business traveler’s may submit a business letter of expedite, if the above is not available.

Fees: - Government and U.S. Vital Document fees can be paid by check, money order or credit card.

• $60 government fee payable to Passport Services
• The U.S. Vital Document fee depends on the processing time you choose, please see the order form for exact fees.

• Do not send your information to any of the addresses on the order form without prior confirmation by our office. We will instruct you in Step 6 (above) where to send your application when you call our office.
